We will bill your insurance provider as an out of network provider only if we are authorized by you. Otherwise, private pay can be arranged. Please speak with the billing department to determine the best choice for you and your family.
Your First Appointment
Please bring with you to the first appointment the following information:
▪ A prescription from your child’s doctor including diagnosis and therapy required ▪ A copy of your child’s most recent evaluation (if available) ▪ Current Insurance Card(s) ▪ Completed Intake Forms
